Transition - Starting School or Early Years
Transition is a term used to explain the move a child makes to start a new phase of education. This could be their first nursery, pre-school or childminder or their reception (Year R) at school.

The Early Years (EY) Transition Service is part of the Services for Young Children (SfYC) Early Years Inclusion team. It supports children with special educational needs at SEN support level whose needs may increase when they move into school. Transition practitioners work with children who may have communication, learning, social/emotional, sensory or physical needs, and work closely with early years settings and other inclusion services (such as Portage and SEN support teams) to make sure the right support is in place.
You can ask for support before your child starts Reception, or schools can request support once children are in Year R.
