Age Concern Hampshire is an independent charity working across the county to promote independent living to older people (over-50s), tackling loneliness and social isolation since 1985. We are passionate about supporting people over 50 to live well independently and support over 5000 older people across the county per year. We use our expertise, knowledge, network and experience to respond to the needs of local people throughout their ageing journey.
The charity currently delivers support from Care & Wellbeing Centres throughout the county, as well as Help at Home, Information Hub services offering free information, Foot care clinics, Hospital to Home services, Community Information Volunteers and a variety of other activities. The dedicated team of fully trained staff and volunteers come from all walks of life to offer friendship, support and information to our service users.
Services offered include:
- Care and Wellbeing Centres - We offer carers and families much-needed support, reassurance and respite by providing day care services for older people who may be living with conditions such as dementia. You can have peace of mind knowing our Care and Wellbeing Centres offer nurturing day care in a safe and secure environment.
- Social Groups - our social groups provide a friendly, comfortable and welcoming space shere older people can feel safe and supported, while enjoying weekly activities and companionship.
- Foot Care - We provide a professional toenail cutting service including regular foot health checks every siz weeks. All appointments are carried out by qualified Foot Health Practitiioners at various locations across Hampshire.
- Help at Home - We provide friendly, reliable help around the home, dedicated to supporting independence and making everyday tasks easier. Activities can be tailored and scheduled as required, including housework, laundry, shopping, meal preparartion, companionship, trips out and dog walking.
- IT sessions - Our IT sessions aim to support older people by enhancing their digital skills, building confidence in using technology and ensuring they stay connected with family, friends and esential services in an increasingly digital world.
- Information and help with form filling - We offer support with completing forms such as Attendance Allowance and Blue Badge applicatins, either over the phone or in the comfort of your own home. We also provide Benefit Checks to help ensure you are receiving everything you are entitled to. In addition, we offer information and signposting for a range of services. - Hospital to Home - In partnership with Hampshire and Isle of Wight Healthcare NHS Foundation Trust our goal is to engage clients during their hospital stay, supporting their safe and efficient discharge from the wards.
