Community NHS Services for Children and Young People
Community health services provide a wide range of support delivered close to home and, where necessary, at home. The aim of these services is to:
- support self-management so people with a health condition can make the most of their life
- help people to maintain independence
- prevent hospital admissions and the need for ongoing care, through prevention and early intervention
- support discharge from hospital and other services to help people recover at home.

Specialist Community Public Health Nurses
Some special schools have Specialist Community Public Health Nurses on site, who help support children with complex health needs and make sure they receive the care they need while at school. -
Community Learning Disability Team
Community Learning Disability Teams support people with a learning disability to stay healthy and live as independently as possible. They work with individuals, their families and carers to understand their needs and make sure they can access the right health support. -

Community Paediatric Medical Service (CPMS)
A specialist children’s health service that assesses, diagnoses and supports children with medical, developmental and additional needs, working closely with families, schools and other professionals. -
The Children's Community Continence Service
The Children's Community Continence Service provides specialist support for children and young people with bladder and bowel issues like constipation, soiling, and wetting.
