The Learning Disability Team supports adults who have had a learning disability since childhood and who may find it difficult to understand information, learn new skills or cope independently. The service works across Hampshire, Southampton, Portsmouth and the Isle of Wight and focuses on helping people stay well, make choices about their health and get the right support.

The team provides community-based health support, working closely with individuals, families, carers and other professionals to understand a person’s needs and support their physical and mental health. This includes help to access healthcare, understand medical information and make decisions about treatment.

People are supported by Community Learning Disability Health Teams, which are based in local areas such as Winchester, Southampton, Portsmouth, Fareham and the Isle of Wight. These teams may include learning disability nurses and other health professionals who work alongside GPs, hospitals and social care.

For people whose needs are more complex, the service can also provide access to Intensive Support Teams. These teams support people whose behaviour may be challenging and aim to help prevent situations from escalating or leading to hospital admission. Referrals to this team are usually made through a person’s local community learning disability team.

The service also provides health facilitation and hospital liaison support. This helps people with learning disabilities receive reasonable adjustments in hospital and ensures hospital staff understand their needs, both during planned care and in emergencies. There are dedicated hospital liaison teams in several local hospitals across the area.
