Getting started with your plan

Use our template to create your own Family Plan to help you manage your health and wellbeing from home with the help of loved ones, friends and your neighbours. 

Look at the Staying independent page for ideas and ways to maintain your independence, and the Community Directory for low cost and no cost organisations that can support you. 

Instructions for use: With the Family Plan open on your screen (a new screen will open in your browser), select ‘Save’ to save a copy to your PC. You can then fill in those parts that are relevant to you. Alternatively, you can print off the ‘My plan’ section and complete it by hand.

Tips for your plan:

  • There are 12 prompt cards below for some ideas.
  • You don’t have to fill in every section, just those parts that you want to change.

Move more and prevent falls

Eat well and drink more

  • Drink plenty of fluids − 6-8 glasses of water unless advised otherwise by a health professional
  • Eat fruits and veg with high water 
    content
  • Eat more protein such as meat, fish, eggs, beans and nuts or try milk powder, seeds, cheese, peanut butter or high protein yogurt for a boost

Priority Services Register

Why will this help me?

The Priority Services Register (PSR) is a free UK wide service which provides extra advice and support, including when there’s an interruption to your electricity, gas or water supply.

How will you do it

Free registration for the priority service register with gas, electricity, and water suppliers. All you need to do is enter your post code on their website.

Who will help you achieve this?

Contact utility suppliers to register for the Priority Services Register.


Information for family and friends

When you’re supporting a friend or loved one, you might feel like you should help them with everything. But sometimes we can do too much, which can unintentionally prevent someone from being as independent as they could be.
